The Form Activity allows course creators to easily create custom forms for pre-assessments, project submissions, and feedback collection. It makes learning more engaging by enabling students to share their thoughts in a structured way. 

Instructors can save time by organizing information more efficiently, and the forms can be used for many purposes, like checking prior knowledge or collecting course feedback. This simple tool makes teaching and learning smoother and more effective.


How to create a form activity element?

Step#1: Navigate to the Curriculum section of your course.

Step#2: Click on the (+ Add Element) button on the left side of the screen.

Step#3: Select Form Activity from the list of available options.

1. Open Question

  • Use this field to encourage detailed responses.

  • Perfect for gathering qualitative insights and fostering deeper engagement.

2. Numeric Rating

(See screenshot below)

  • Enable reviewers to rate by selecting a number.

  • Ideal for assessing understanding, satisfaction, or task performance.

  • You can also add additional comment to it in which learners will explain they selected that numeric rating.

3. Multiple Choice

  • Allow reviewers to choose from a list of options.

  • Simplifies decision-making and streamlines knowledge testing.

  • You can also add additional comment to it in which learners will explain they selected that choice.

4. Stack Ranking

  • Let learners rank a list of options based on their preferences or priorities.

  • Useful for understanding what aspects of your course resonate most with learners.

  • You can also add additional comments to it in which learners will explain they selected that stack ranking.

5. Information Section

  • Provide essential guidance or background information to reviewers.

  • Helps clarify the purpose of the form and aligns expectations.

Getting Started with Form Activity

The first thing you will see after clicking the form activity are the common settings. After going through the common settings of the form activity we are ready to hop on to the next part.

2. Customizing Your Form

  • After scrolling down from the common settings, click on the Edit Questions button.

  • Add questions by choosing one of the following field types:

    • Open Question

    • Numeric Rating

    • Multiple Choice

    • Stack Ranking

    • Information Section

3. Editing Questions

  • Use formatting options (bold, italic, underline) to emphasize important details.

  • Customize field types to suit your activity goals.

Optional Settings

  • Edit Option: Allow learners to edit their answers after submission.

  • Enable or disable this option depending on your course requirements.
